On-line CE Evaluation & Claim Credit Process
Please note that the Pharmacy Foundation’s CE evaluation & claim credit service will no longer accept voucher codes for Outlook 2010 programs.
However, you can still access Statements of CE Credit from Outlook 2008 (Sacramento), Outlook 2009 (Disneyland) and/or Outlook 2010 (Long Beach). Click here to access the CE Evaluation & Claim Credit Portal. Once you are on the “Welcome Page”, click the “login” link (on the left) and enter your login and password. On the “Personal Page”, click on the “History” link (on the left). Your “History Page” will show all courses you have completed and there will be a “Print” link next to the Statements of CE credit that are available. Note: If you have printed the Statements of CE Credit in the past, the word “Duplicate” will appear on the Statement. A detailed instruction manual (with screen shots) is available - click here.
