Jane Boggess Advancement of Pharmacy Practice Award
This award recognizes an individual or organization that has significantly contributed to the broad adoption of one or more non-traditional pharmacy services which improve public health. The recipient will receive a complimentary registration to the Outlook 2010 pharmacy conference where they will be honored with an engraved crystal award at the Pharmacy Foundation of California’s annual Leadership & Awards Breakfast.
This award was established in 2003 but the name was changed in 2004 to honor the memory of the award’s first recipient, Jane Boggess, who championed the effort to enable pharmacy access to emergency contraception, and unexpectedly passed away before receiving the award.
Criteria
Individuals or organizations, from within the pharmacy profession or outside of it, who have significantly contributed to one or more of the following:
Ø Achieving broad recognition of the value of one or more non-traditional pharmacy services.
Ø Identifying and overcoming barriers related to a pharmacist’s provision of such services.
Ø Supporting pharmacists in their efforts to offer these services by providing them with information,
education, and/or practice tools and resources.
Nomination Process
The nominator is responsible for submitting an award nomination to PFC by October 23, 2009*. Nomination materials must include the following in order to be considered:
Ø Official nomination form.
Ø A letter from the nominator explaining how the nominee meets the award criteria and specifically
documents why the nominee is deserving of the award.
Ø A copy of the nominee’s resumé.
* Nominations sent via email must be received by the deadline date and be sent to spresidio@pharmacyfoundation.org. Nominations sent via mail should be postmarked by the deadline date and sent to: Pharmacy Foundation of California • Attn: Awards & Scholarship Division • 4030 Lennane Drive • Sacramento, CA 95834. Faxed and late submissions will not be accepted.
Selection Process
Staff will review all nomination materials submitted by the deadline for completeness and conformity with the nomination guidelines. Incomplete nomination materials will not be considered by the PFC Awards Committee and will be returned to the nominator. Acceptable application materials will be forwarded to the PFC Awards Committee for evaluation and selection, with ratification by the full PFC Board of Directors.
